Forms for Your First Appointment

Your first visit to Angel Hands Physical Therapy LLC should last approximately an hour and a half. Please wear comfortable clothes and bring your insurance card, photo ID and any written MRI and / or X-ray reports. You will receive a full evaluation through questioning, a physical exam, and a test of your range of motion. Your medical history, diagnostic tests, and any recent events that may have contributed to your current condition will be reviewed.

Your evaluation will include an assessment of your current functions, pain level, and posture, as well as an evaluation of your flexibility, strength, endurance, and balance. It is also recommended that you receive a referral from your primary care doctor or orthopedic surgeon.

Please fill out the forms below ahead of your visit and email completed forms to danielle@angelhandspt.com. Or fill out forms, print and bring them with you. If you are unable to, please arrive 15 minutes early to your appointment to fill them out in our office.

  • Will I see the same therapist at each visit?

    Yes, Danielle is our only physical therapist at Angel Hands Physical Therapy, as well as the owner.

  • What should I bring to my PT appointment?

    Please bring a written X-ray report and / or MRI report.

  • How frequently should I come for physical therapy?

    Two to three times a week if it is covered by insurance. One or more times a week if paying out of pocket with cash.

  • What is a physical therapist?

    A physical therapist is a health care provider who is considered a movement specialist and uses hands-on techniques, exercises, and modalities to relieve pain and restore function.

  • How long are your appointments?

    The initial appointment will last approximately 90 minutes. After that, each visit should be for about an hour.

  • Do I need a referral from my primary care doctor for receiving physical therapy?

    If you are planning on paying cash for your therapy, you do not need a referral. If you are covered by insurance, you will need a referral.

  • What should I wear to my PT appointment?

    You should wear comfortable clothes, such as sneakers or shorts.
